To make a formal referral this needs to be done by a placing authority. If you are a parent or family member you will need to speak with a social worker to do this.
Our organisation is an approved, contracted and framework provider of supported living services by local authorities in the locations to in which we operate services.

We will ask the placing authority to share documents detailing the persons support needs. The Management Team will review these documents, and see if they feel the needs of the individual can be met by our organisation. If in principle they feel they can, then an assessment will be offered. The team will then offer to meet with the individual in order to carry out an assessment.
During the assessment process, information will be gathered from people who know the induvial best, such as family members and people currently supporting them. As part of the process we will study and consider the person’s support and health needs. If we feel we can meet the needs of the individual, then a placement will be offered. We will then put together a costing based on the individual needs of that person and we will offer a place to the appropriate authority.
Subject to the placing authority confirming that they accept the proposal, we will offer to plan a transition. We understand that this transition can represent a difficult and stressful time for anyone, especially as moving into a new home is particularly difficult if that person has autism or does not like change. A new environment and meeting lots of new people can therefore be a distressing and challenging time. It is therefore key that all interested parties work as closely as they can to help and assist one another during the transition process.
The transition process will be managed by our staff team. We will involve the individual, parents, family member and professionals to ensure the move is planned and managed in a person-centred way. We want the individual who we are going to be supporting to feel as comfortable as possible. We will initially create opportunities to meet the staff team. They will then progress to meeting people who live at the property already (if applicable) and to spend some time personalising their new accommodation so it feels like home as soon as they move in.
Occasionally, circumstances may arise when a placement needs to be offered urgently. We endeavour, where practical and possible, to apply the same approach and offer support to ensure the transition is as smooth as possible.
We always aim to ensure the people who choose our support are as happy as possible from the first day they move in.
